If you require campus housing through the remainder of the Spring 2020 semester (March 30th through May 3rd), please submit the following information.

*Note* Students who previously were approved to stay on campus during the recent two week closure (March 14th through March 29th) MUST also complete this form.

This is an application. Approval of a request is not automatic. You will be contacted by email to confirm the status of your request. We may ask for documentation to support your request.

I understand that as the situation changes and continues to unfold, that Housing & Residence Life reserves the right to end or remove my stay on campus if determined necessary for Health, Safety, or Conduct related reasons.
